[Note: The forms are formated in Excel, with 15 pages to the Adult Intake and 16 pages to the Minor Intake. You must print all pages by selecting the page tab at the bottom of the spreadsheet, or you can print all pages at once by holding down the shift key and selecting the tab labeled page 1 and then selecting the tab labeled page 15 or page 16 while still holding the shift key down (this will highlight all pages for printing.)]

Complete the approriate intake package and bring the information with you to your first session.  Adult Intake 18+ years of age; Minor Intake 17 years or less.  One of our office staff or your therapist can go over any questions you might have regarding these forms at your first session or you can contact us at (559) 738-0700.
